NOTES:
 

• Unless otherwise specified, new patient visits are 60 min and established visits are 30 min. 

• Labs tests -including blood work, pap cytology and tissue pathology can all be separately billed to insurance by the receiving lab. 

• Telehealth services can be utilized at your convenience with Elation’s HIPAA-compliant integrated video platform, powered by Zoom, for appropriate visit types not requiring a pelvic exam. Fees are the same as in-person office visits.

• If a procedure can be added to another office visit same day, a $50 discount will be applied to the procedure fee. 

• A payment method on file is required to keep your appointment reserved. Given we do not accept any payments from insurance companies, fees are fully transparent and due at the time of service. You will always be notified before a charge processes and receive a receipt after. No charges are incurred until services are rendered, but there is a fee assessed for "no-show" or late cancellation within 48 hrs. This fee is $50.
